New Patient Forms
Please complete the following forms prior to your first appointment at Grand Primary Care. Completing these forms ahead of time will save you a significant amount of time checking in for your first appointment. You can print off the forms and bring them to your first appointment, or you can fax the completed forms to (833) 974-5242 prior to your appointment.
If current patients have urgent medical questions that you feel cannot wait until the next business day, please call the main office phone (616) 333-5913 and follow the prompts to leave a voicemail for the on-call provider.
The on-call provider will return your call within 30 minutes.
If you have an emergent or life-threatening situation, please bypass the on-call provider and call 911 or go directly to the nearest emergency room.
